Upon purchase, you will receive an email with a download link to the Trunk Show Host Consultation Package — a 3-page PDF document containing the Host Inquiry & Scheduling Form and a summary of trunk show terms.

Please read the document carefully, complete all sections, and sign the acknowledgment. You may print, complete by hand, and scan the form, or open it in Adobe Acrobat to use the built-in text and signature tools. Return the completed form to couture@stacystewartsmith.com with the subject line: TRUNK SHOW INQUIRY — [Your Organization Name].

Once we receive your completed form, we will contact you within 3 to 5 business days to confirm your consultation date and time.

The $20.00 fee is non-refundable. Incomplete forms will not be processed. We do not accept unsolicited phone inquiries.
